This form is used to submit a loaner request to use an MRD5 using BoilerConnect or BoilerTrack.

MRD5 readers are available for short-term, event-specific use by Purdue departments and units. Requests must be submitted at least 24 hours prior to the event. Same-day requests cannot be accommodated.

Due to high demand and limited inventory, departments may request a maximum of three (3) loaner MRD5 devices per event.

MRD5 devices configured for BoilerConnect or BoilerTrack are programmed with a single leading zero (0) in the output format.

Loan Period: MRD5 readers may be borrowed for up to three (3) business days. Devices may be picked up the business day before the event and must be returned the business day after the event. Weekend days do not count toward the three-business-day limit.

Program Guidelines: MRD5 readers are available for short-term, event-specific use only and are subject to availability. Departments with ongoing or recurring needs should purchase their own device. Failure to return equipment by the designated return date may result in the loss of future borrowing privileges.
