Separation/Termination Request

This form is used to separate/terminate one or more appointments.

Please Note: We cannot undo any separation after the request is fully approved in Successfactors. If a separation request is submitted in error, a rehire request will be needed. The employee may be required to complete the onboarding process again.

You will need to include the reason for the separation. Reasons include the following:

Career Opportunity, Contract not renewed, Death, End Additional Assignment, Inadequate Performance, Job Abandonment, Leave Exhausted, On Boarding Noncompliance, Personal Reasons, Reduction in Force (RIF) please attach RIF letter, Resignation in lieu of Termination, Retirement (55+ year of age and 10+ years of service required), Temporary or Seasonal, Work Authorization
