Overview
The Purdue Veterinary Hospital Central Supply office is now using TeamDynamix to more efficiently meet the needs of hospital staff. To submit a requisition or equipment repair request, please follow the instructions below.
Instructions
There are two different ways to submit a request:
- You may email pvmcs@purdue.edu with your request.
- Use the link below to select either a Requisition form or an Equipment Repair form.
- Click the link for a button to a request form. The link will also show your recent requests.

- Choose the "Veterinary Hospital - Central Supply Requisition Form"

- You will be prompted to sign in with your career account.

Whether you email us or fill out the request form online, please make sure to:
- Let us know what department this is for
- When you need it
- Add any links or attach any extra information
- If this is for research or a class, please add your C ID# or F account #
