How to create a Veterinary Hospital - Central Supply request

Overview

The Purdue Veterinary Hospital Central Supply office is now using TeamDynamix to more efficiently meet the needs of hospital staff.  To submit a requisition or equipment repair request, please follow the instructions below.

Instructions

There are two different ways to submit a request:

  1. You may email pvmcs@purdue.edu with your request.
     
  2. Use the link below to select either a Requisition form or an Equipment Repair form. 
     
    1. Click the link for a button to a request form.  The link will also show your recent requests.

    2. Choose the "Veterinary Hospital - Central Supply Requisition Form"

    3. You will be prompted to sign in with your career account.

Whether you email us or fill out the request form online, please make sure to:

  • Let us know what department this is for
  • When you need it
  • Add any links or attach any extra information
  • If this is for research or a class, please add your C ID# or F account #
